Remote Patient Monitoring (RPM)

Stay Connected With Your Care Between Visits

Remote Patient Monitoring allows your healthcare team to follow important health information while you are at home. Connected devices can securely transmit readings such as blood pressure, blood glucose, heart rate, weight, or oxygen levels, depending on your individual care needs.


RPM can help your provider identify meaningful changes, follow your progress, and provide additional support between office visits.

Conditions That May Benefit From RPM

Remote monitoring may be helpful for patients managing chronic conditions such as:

If you are enrolled in RPM, you'll use a connected monitoring device appropriate for your care plan. Your readings are securely transmitted so your healthcare team can review the information and follow important trends over time.


Depending on your needs, your care team may use this information to follow your progress, discuss concerning changes, and make adjustments to your care plan when appropriate.

Care Beyond the Office Visit

RPM gives your healthcare team additional information about how you're doing between appointments. Instead of relying only on measurements taken during an office visit, your provider can review health information collected while you're at home.

Insurance Coverage
